Help with C1AZ-17696-A (mirror)

Been having trouble identifying the make and model for this mirror.
I've had it over 30 years, before that it was my dads.
I've looked through the net and have some possible leads:
Galaxy 500 XL
Fairlane
Falcon
1965-1966???
I've looked online for those models but none I've seen show this mirror.
Any help is appreciated

The C1AZ number tells all, first used on 61 full size cars but may have been used for several years and/or models.

I have found the "A" is for full size cars such as the Galaxie.
Still tying to track which models they stuck them on. -
